• Housing Log of places the applicant applied BEFORE you started using HousingWorks 

• Applicant’s Housing History for last five-ten years 

• Proof of current income 

• Two-character references, notarized if possible 

• Proof of extraordinary medical expenses, show proof 

• Proof of a documented disability 

• Social Security Card or ITIN proof. 

• Letters from Welfare offices, EBT, etc.
